Social work services

Cass Regional's Case Management/Social Work services staff members assist with discharge planning needs. They also provide information about community resources should a patient or family member need help with emotional concerns, support from others with similar problems or help with making arrangements for post-hospital care. Our Case Management/Social Work department is a great resource to contact if you need information about durable medical equipment, transportation, disability, legal advocacy, private home care, home health, hospice, advanced directives, rehabilitation and much more.

Case Management/Social Work services is available Monday through Friday, from 8:30 a.m. to 5 p.m.
