Cass Regional is committed to protecting your personal health information. To request your personal medical records, please submit an online electronic request or print out the form(s) required. Helpful instructions are provided to assist you in obtaining your records. If you would like assistance with completing the form(s), our Medical Records staff is available to speak with you Monday through Friday, from 8 a.m. to 4:30 p.m., at 816.887.0771.
We are dedicated to ensuring your records are kept in strict confidence and are not released without your consent, except as required by law.
